Plan of Delivery.


Obtaining care for your loved one, friend, client, or yourself is easy! Below is a summary of steps to help you understand the process. The road to independence and freedom is just a couple of steps away!



  Step 1: After completing our simple form online or calling us, our professional support staff will follow up with the client to set up a free initial functional assessment at a time that is convenient for the client and their family members. An Individual Service Plan will be initiated at the time of initial evaluation. The ISP will be developed after consultation with the client/ or client’s representative and shall include potential services to be provided and the requested hours of service and the cost of the services for hourly, weekly, per diem, live-in and the method of reimbursement will be established before  start of care.


The ISP for each client will be communicated to the caregiver prior to or at the time of the delivery of care. The ISP shall be revised as necessary, but it shall be reviewed and updated by the supervisor designated for supervision of the client services at least every 60 days as required by AHC and six months by Oklahoma regulation (2455 eff 7-11-2008)


Step 2: During the initial meeting, an individual service plan at the time of the initial evaluation shall be established.  A safety evaluation will also be performed to ensure safety. During this meeting with the client and family members, our Professional Home Care representative will go over the services, schedule, and pricing that is needed for the care.


Step 3: Once a plan is set, an agreement is signed between both parties. This document outlines services, schedule, and pricing. A start date and time will be established.


Step 4: After signing the agreement, we match a Caregiver to the client based on their unique and specific needs. Skill level, Experience, and Personality all play a role in choosing the right Caregiver for the client.


Step 5: In order for the Caregiver to fully understand their assignment, we cover the approved written care plan with them. Care supervisor will orientation/shadowing/coaching of the caregiver will be implemented and established by the site supervisor/manager. Caregivers will understand the specific care and needs of the client before they are left alone at the client’s home. Caregiver MUST fully understand the specific needs, requirements, and special circumstances of clients.


Step 6: In order to ensure on-going quality, our quality assurance team makes scheduled and un-scheduled visits when the Caregiver is present and not present on a bi-weekly basis or whenever necessary. There, we re-evaluate and re-assess any changing needs of the client. At Home Care, Inc. is dedicated to provide on-going quality service.


Step 7: Information to file concern or complaint provided to client and client’s representative. 24/7 phone service is available to clients and representative.
