We provide a complete continuum of care to individuals, older adults, and frail older adults and disable who are homebound and have a skilled medical need. We offer the following services to our clients: Nursing care, Home health aide, Occupational therapy, Physical therapy, Speech therapy, and Social work.
Admission Criteria: The patient must be under the care of a licensed physician, a skilled service must be ordered, be confined to the home
•Require skilled nursing or therapeutic services on an intermittent basis
•Requires face-face within 90 days of receiving home care services or within 30 days of
•Receiving the start of care.

Skilled Nursing
Part-Time intermittent skilled nursing services – comprehensive Assessment, daily and weekly home visits and emergent care. Skilled nursing care can include physical exam assessment, wound care and dressing changes, catheter care, IV therapy, colostomy care, and patient and family teaching. The nurse coordinates your care at home and keeps the doctor informed of your progress.
Physical Therapy
A licensed physical therapist assesses the patient's ability to function in the home by evaluating strength, range of motion, endurance, coordination, balance, pain, posture, and mobility. The therapist develops and implements a rehabilitation program which will help the patient maximize total function.
Occupational Therapy
A licensed occupational therapist assesses the patient's ability to perform activities of daily living and fine motor functions, the need for adaptive equipment in the home, and the patient's cognitive and perceptual function. The therapist develops a program geared to maximizing safety and independence.
Speech-Language Therapy
A licensed speech-language pathologist assesses and treats speech, language, and swallowing disorders. Specific treatment programs are developed for aphasia, voice disorders, aphasia, dyspraxia, dysphasia, speech, and language in order to maximize the patient's communication and/or swallowing skills.
Medical Social Work
Licensed clinical social workers (LCSW and MSW) participate in the development and implementation of the plan of care by assessing social factors which impede recovery and coordinate use of community resources and special support groups. Social workers also counsel patient or family members if there is a need related to loss of health and function and/or a need for psychiatric services.
Home Care Aide
Home care aides provide or assist patients with such personal care as bathing, dressing, oral hygiene, meal preparation, feeding, walking, and exercise. These services are designed to help patients regain maximum independence in providing self care.\
Payment Plans
Home health care services are covered by Medicare, Medicaid, Blue Cross, Tricare, and most commercial insurance policies. We also honor private-pay. To be eligible, patients must:

•At Home Care Home Health Care, LLC offer these services:
Home Health Nursing Services are initiated by a registered nurse or therapist who develops the plan of care with you, your doctor, and your family. Skilled nursing care can include physical exam assessment, wound care and dressing changes, catheter care, IV therapy, colostomy care, and patient and family teaching. The nurse coordinates your care at home and keeps the doctor informed of your progress.
Home Health Aide Services:
•Supplements the care provided by the registered nurse by helping with daily needs. Services include assistance with personal care: bathing shampooing, and changing linens. The aide also can help with meals, simple prescribed exercises and limited household tasks.
Physical Therapy Services :
•Are provided to relieve pain, prevent further disability and restore function for maximum independence through a home exercise program designed by a physical therapist.
Occupational Therapy Services:
•Teach patients how to adapt daily routines and surroundings for more independent living. Occupational therapy includes instruction in self-care and homemaking tasks.
Speech Therapy Services:
•Help with problems with pronunciation, swallowing or hearing and can help with the development of language skills.
Medical Social Services:
•Help with personal, family, and financial problems that may develop as a result of your illness or disability.
Medical Supplies:
• Such as dressings, tape, and catheters are available from the home health agency. Incontinent supplies--diapers and choux--also can be obtained and more.
Equipments:
•Such as a wheelchair, walker or hospital bed, if needed, can be obtained with
assistance from the home health agency.
•Other Services: Medication Education and management, Diabetic Education and Monitoring, Home Health Aides to Assist with your Personal Care, Homemaker services, Wound Care Management, Balance and gait deficits, Assertive devices, Unstable blood sugar.
